How old does my child have to be?

Ages 3 to 5.

Who is eligible?

In general, eligibility is based on family income.  We strive to serve all by getting the opportunity to meet with all families in person.  

How do we apply?

1.  Get an Application Packet by mail, in person, or download a copy by clicking on each button below:

   

    a.  Head Start Applicant Checklist for Parents form     

   

    b.  Applicant Information form    

   

    c.  Family Information  form

                                               

    d.  Health Appraisal form

                                                           

    e.  Dental Appraisal form                                                           

 

2.  Fill out the application packet.

3.  Call 505-867-3351 Ext. 121 to schedule an Enrollment Appointment with the Family Services Coordinator, who will review your completed application packet with you to determine eligibility.  If you are eligible, you will complete enrollment for your child.    

Please be aware of the following during your application process:

  • An incomplete application packet will delay your application process.

  • A complete packet does not guarantee enrollment due to reasons to be determined by the program, such as eligibility guidelines and classroom capacity.

The Five Sandoval Head Start Program aims to strengthen the development of children from ages 3 to 5 through a comprehensive approach.  We nurture the development of each child and their family with important focus on developing their strengths and needs.  We strive to empower  and enhance our children to become independent and self-confident, as well as be inquisitive learners by supporting them with effective educational, social, nutritional and health related services.
